The Amulet of Betrayal Personified is a unique artifact introduced in the Mask of the Betrayer expansion for Neverwinter Nights 2. The effects of the amulet are indeed one of the most impressive in the game.

Penalties to saves while using this item are severe, but multiclassed characters with high charisma bonuses to saves (such as Blackguard/Paladin or Warlock/Blackguard) can adequately overcome them, making ongoing use of this amulet a realistic possibility - especially with the maximum bonus to charisma it provides on its own. For example a Warlock/Blackguard would, absent additional enhancements, still have a net +3 to saves wearing this amulet (this includes taking into account the +12 to CON, DEX and WIS it provides).
